Welcome to the Smithtown Slammers Travel Return to Soccer Team Manager / Trainer Page!

It’s great to be able to get back on the field!

This form is for current trainers and team managers to fill out and agree to the release waiver. It is imperative that you read the entire waiver and abide by the COVID-19 rules at all times.

If you are a current player or are interested in joining the Smithtown Slammer family as a player, please use the Travel - Return to Soccer - Players page.

If you do not fill and agree to the waiver, you WILL NOT be able participate in any club activities.

Please note that any reference to "players" is should be understood as Team Manager or Trainer. We are trying to use existing tools to capture all our sign-ups in the quickest manner possible.
